ADULT DAY HEALTH
The mission of the Adult Day Health Center is to provide therapeutic physical, mental, and social activities using an interdisciplinary team process to help clients restore or maintain optimal capacity for self care and to allow them to delay institutionalization.

Program Description

The Leahi Adult Day Health Center is a program designed to provide therapeutic services and treatments which include nursing, health care, group activities, socialization, recreational therapy and social services for those who may be at risk for long term care placement.

Our primary objectives are:

  • Restore or maintain optimal capacity for self care
  • Delay institutionalization
Services Offered

A plan of care is developed using a multi-disciplinary approach to meet each clients special needs.
Available Services Include:

  • Nursing Services
  • Recreational Therapy
  • Social Work Services
  • Health Education
  • Dietary Services (Lunch and 2 snacks per day)
  • Bathing
  • Consultative Services such as Occupational Therapy and Physical Therapy may be available when appropriate.
Hours of Service

Monday through Friday
7:00 A.M. to 5:30 P.M. (last Handivan pickup is 4:00 P.M.)
Closed on weekends and State Holidays

Eligibility

Clients admitted to LADHC must meet the following criteria:

  1. Be 18 years of age and over.
  2. Be ICF certified by a medical doctor, which means the physician has certified the client as having health problems which meet the criteria of the Intermediate Care Facility level.
  3. Possesses the strength and endurance to participate in an activity program and the willingness to strive for a higher level of independence as determined by the Center.
  4. Have an interested family member or caregiver at home, who is committed to supporting the Centers goals for the client.
  5. Have a complete physical examination and a two step TST (Tuberculin Skin Test) or chest x ray taken within the last twelve months prior to admission and annually thereafter.
  6. Be free from any communicable or infectious disease.
  7. Behavior that is acceptable to the functioning of the Center.
Fees

Rates for our service are tiered with levels I through III based on the needs of the individual. The level of each participant will be determined at the pre-admission assessment. Level I fee is $80.00 per day.
Leahi Adult Day Health Center has a contract with the State of Hawaii Dept. of Human Services to accept the Medicaid Waiver from Nursing Home Without Walls.
