JOB DUTIES: This position is located in the Health Information Management of Kauai Veterans Memorial Hospital (KVMH) and the Samuel Mahelona Memorial Hospital (SMMH). The Coder III is primarily responsible for performing routine qualitative and quantitative chart analyses to ensure complete physician documentation and indexing, coding assignments for reimbursement purposes, prompt transmittal of patient discharge data to Patient Financial Services Department, ongoing scrutiny of compliance with record completion procedures, release of medical information to third party carriers, release of information to others as dictated by policy and procedures, and other duties as assigned. This position may be required to work shifts, weekends, and holidays.

MINIMUM QUALIFICATION REQUIREMENTS:

EDUCATION: High school diploma or equivalent.

EXPERIENCE:  Specialized Experience: Two (2) years of experience in a health information management department of a hospital or allied medical care facility which involved the performance of paraprofessional health information management work requiring the knowledge of the nature and uses of medical records, medical terminology and established health information management classification, coding, filing and indexing systems; and the ability to analyze and file medical records and prepare reports and summaries from medical records.

Coding Experience: Two (2) years of experience in a health information management department of a hospital or allied medical care facility, which involved the performance of assigning codes from the listing of ICD-9-CM, CPT-4 and HCPCS for all diagnoses and procedures and other services. The experience must have been gained within the last five (5) years.

Certification: Valid CCS (Certified Coding Specialist – hospital) [inpatient coding] from AHIMA (American Health Information Management Association), or Dual certification – CCS (Certified Coding Specialist – hospital) [inpatient coding] and CCS-P (Certified Coding Specialist – Physician-based) from AHIMA (American Health Information Management Association) is required.

Mobility: Move freely throughout the work setting and respond to emergency situations quickly.

DESIRABLE QUALIFICATIONS: Ability to handle emergency situations in a calm manner; ability to communicate and work effectively with distraught or agitated individuals.

FUNCTION AND LOCATION:  This position works in the respiratory therapy unit of a hospital and exercises considerable independent clinical judgment in applying scientific knowledge and theory to practical respiratory care problems in accordance with physicians’ orders.  The position is responsible for independently administering respiratory tests and treatment to patients with respiratory diseases and disorders including cases involving critically ill, pediatric, and neonatal patients.  The position may be required to work on shifts, including evenings, and weekends and holidays.

Experience and Essential Knowledge and Abilities: One (1) year of progressively responsible clinical work experience which demonstrated competence in independently administering treatment and/or testing procedures in pulmonary functions, blood gas analysis, critical care including mechanical ventilation, and pediatric/neonatal respiratory care.

Substitutions allowed: Completion of a four (4) year college or university accredited program in respiratory therapy resulting in a Bachelor of Science degree may be substituted for all of the required experience.

EDUCATION LEVEL: 2-Year Degree

FUNCTION:   This position is located in the Emergency Department and the Acute/Swing Bed Unit of Samuel Mahelona Memorial Hospital (SMMH).  SMMH is part of the Kauai Region, Hawaii Health Systems Corporation, a public benefit corporation of the State of Hawaii operating 13 facilities statewide.

The Emergency Department (ED) provides nursing care and first aid to patients requiring emergency services; assists the physician in the examination and treatment of patients; prepares and maintains records of all emergency cases; and arranges admittance to the hospital as required; and for outpatient surgery patients, provides preoperative and postoperative care.

The Registered Professional Nurse III (RPN III) provides professional nursing care within the ED; coordinates care with other disciplines.  Provides care that meets the psychosocial, physical and general aspects of care.  Determines priorities of care based on physical and psychosocial needs of patients, as well as factors influencing patient flow through the system.  Communicates with the ED physicians regarding changes in patient’s status, symptomatology and results of diagnostic studies.  Responds quickly and accurately to changes in condition or response to treatment. Position is subject to shift work and may be assigned to cover other nursing units as needed.

The RPN III works under the general supervision of the attending physician and reports to the Director of Nursing (DON).

FUNCTION:   The Coder II is primarily responsible for performing routine qualitative and quantitative chart analyses to ensure complete physician documentation and indexing,  coding assignments for reimbursement purposes, prompt transmittal of patient discharge data to Patient Financial Services Department, ongoing scrutiny of compliance with record completion procedures, release of medical information to third party carriers, release of information to others as dictated by policy and procedures, and other duties as assigned. This position works closely with Utilization Review and may be required to work shifts, weekends, and holidays.

MINIMUM QUALIFICATION REQUIREMENTS:

Education: High school diploma or equivalent.

Specialized Experience: One (1) year of experience in a health information management department of a hospital or allied medical care facility which involved the performance of paraprofessional health information management work requiring the knowledge of the nature and uses of medical records, medical terminology and established health information management classification, coding, filing and indexing systems; and the ability to analyze and file medical records and prepare reports and summaries from medical records.

Coding Experience: Two (2) years of experience in a health information management department of a hospital or allied medical care facility, which involved the performance of assigning codes from the listing of ICD-9-CM, CPT-4 and HCPCS for all diagnoses and procedures and other services. The experience must have been gained within the last five (5) years.

Certification: Valid CCS-P (Certified Coding Specialist – Physician-based) from AHIMA (American Health Information Management Association) or CPC or CPC-H (Certified Professional Coder – hospital) [outpatient coding] from AAPC (American Academy of Professional Coders) is required.

Licensure: None required.

Desired skills: Working knowledge of International Classification of Diseases, 9th Edition, Clinical Modification (ICD-9-CM), American Medical Association, Current Procedural Terminology (CPT-4) Coding system, and Healthcare Common Procedure Coding System (HCPCS); nature and uses of medical records; medical terminology; knowledge of disease processes and pharmacology; established health information management classification, filing and indexing systems; the use of computer equipment and software applications; strong analytical skills and office practices and procedures.

Ability to analyze medical records; prepare reports and summaries from medical records; review, interpret and apply established health information management policies, guidelines, standards and procedures; perform arithmetic computations; proficient use of computers, word processing and other software applications; follow oral and written instructions, and deal tactfully and effectively with others. MAJOR DUTIES & RESPONSIBILITIES: 

This position serves as the Director of Risk Management.  This position oversees a comprehensive, system-wide risk management program.  In doing so, the Director of Risk Management develops, implements, and evaluates the risk management program for the HHSC system, in conjunction with the Regional and Facility Risk Managers, and under the direction of the Chief Operating Officer.

  • Design and implement a system-wide, integrated delivery system model risk management program.
  • Design and implement risk management education for all employees and the medical staffs, including hospital, long term care, and clinical sites.
  • Design and implement a system for risk management data aggregation, analysis, and information sharing on loss prevention and loss control, to include appropriate benchmarking.
  • Review and revise existing risk management related policies and review other policies for risk issues.
  • Assist Chief Operating Officer in overseeing the insurance program for the system, which includes applications for purchase of insurance and managing the insurance process throughout the policy year.
  • Assist Chief Operating Officer in overseeing the claims management process, including direct management of claims arising from terminated operations in the absence of a responsible regional or facility risk manager.
  • Participate in disaster planning and act as a core member of the command and control team during a declared emergency.
  • Act as a resource to the Facility and Regional Risk Managers on day to day risk management related questions and requests for assistance as they arise.
  • Work with Regional Quality Directors and other appropriate personnel to coordinate patient safety, performance improvement, and other system wide activities involving risk of harm or loss.
  • Assist the facilities with risk management issues relating to accreditation and licensing, serving as a resource to the facilities as necessary on Joint Commission and other standards.
  • Assist the facilities in training new risk management personnel.
  • Provide on a quarterly basis to the HHSC Board via the Quality Improvement and Patient Safety Committee and other boards as requested a report on risk management activities.
  • Plan and conduct periodic meetings with all the system risk managers in order to exchange ideas, issues, and to provide a training forum.
  • Provide, upon request and time permitting, risk management support of the types described above to HHSC’s subsidiary ambulatory surgery center and at 501(c)(3) hospital and clinic entities affiliated with HHSC.
  • Act as the liaison between the DAGS risk management program and our facilities.
  • Establish and maintain courteous, cooperative, and collaborative relations when working with personnel in the regions, the Corporate Office, subsidiaries and affiliates, and the public.
  • Perform related duties and responsibilities, as assigned.

Requirements

Education Requirement:  A Bachelor’s, masters or doctoral degree in a healthcare profession, health administration, or public administration, or a law degree.  A law degree is preferred.

Specialized Experience:  This position requires a minimum of five years of experience in acute hospitals and/or integrated delivery systems, and a broad knowledge of hospital operations.  Position requires extensive experience in risk management, which may take the form of practicing law in a medical malpractice defense practice (preferred) and/or performing risk management and/or patient relations in an institutional setting.

Certification:  ARM, CPCU, GASHRM, DFASHRM, or CPHQ or CPHRM designation is desirable.

License: None required.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

This position requires post-graduate level competencies in statistical analysis.  Position requires excellent analytical, organizational and communication skills, and the ability to handle multiple projects at the same time.  Position requires a healthcare administrative or clinical background, preferably in a hospital or healthcare facility setting, or as medical malpractice defense counsel.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:

The position will work in an office environment with prolonged sitting.
